Remove basictracer package #86

The lightstep/basictracer package is a leftover from when we depended on opentracing/basictracer. Now that this code is part of the lightstep tracer codebase this factoring no longer makes sense, and could be simplified by dissolving this sub-package into the main lightstep package.

This work is completed in v0.13.
