This PR is the no-protobuf companion PR to https://github.com/lightstep/lightstep-tracer-javascript/pull/243.

Reviewers, please scrutinize this as this hasn't been released in a little while and the merge and resolve conflicts phase was a pretty delicate process with lots of margin for error.

Hey @mwear re-posting here just in case. Posted something similar in the master branch pr. We missed changing the index.d.ts files to account for our nodejs_url change. Here's the pr to do so #245
