HTML Named Entities substituted in XML doc code examples #87

Hi,

I have a piece of XML documentation

And the '>' in the lambda expression is rendered, literally, as > by Vsxmd in the resulting MyProject.md:

The expected behavior is for Vsxmd to treat a <code></code> block as just that -- pre-formatted code -- and not change dingbats to Named HTML Entities.
