Establishing associations finalized #65

Implemented point 11 from the spec, specifically:

added bind
added getValidated3pid
added unbind
added new table 'mappings' to db to handle these three endpoints
added attempt handling to requestToken and adapted it to work with the other functions
adapted submitToken to work with other functions
added functions to sign JSON objects from spec Appendices 3.1 and 3.2 to twake/crypto

For each of these functions, tests were added to make sure they work correctly and that the minimum coverage is met.
