I'm opening this up as a draft for some general fixes and improvements related to piet-cairo.

I'd be happy to get this in as-is, seems like a nice isolated patch?

I'm just currently trying to debug RTL FIXMEs (to see if the hacks around them are even still relevant). Specific diving into pango for the issue around x_offset for RTL. Feel free to merge as is, I'll just make another with the potentially the same name with more changes
