Add some docs.

I made these notes as I was learning my way around. You may want none, some, or all of these. Let me know which if the answer is "some" and I will remove the others.

In the latest diff, could you check that I've used the terms code point and grapheme cluster correctly.

Sorry about the wait, I was a bit intimidated by my lack of understanding. Ready for review again. I think my preference is to merge it and then tackle issues with it individually, so I can keep the amount I have to have in my head manageable.
