Hi there!

I needed to import invoices in QuickBooks and I am using some fields that were not supported by the DSL:

Shipping address (SADDR1 to SADDR5)
Sales rep (REP)
Invoice title and memo (INVTITLE and INVMEMO)
Purchase order number (PONUM)

This commit adds them and also updates the specs.

BTW, thank you for this project! It made it really trivial to generate the IIF files I needed :)
