add some examples of app/addon/engine usage #222

As I make some reproduction test cases, I figure I would document behavior in the readme in the form of examples. Hopefully showing folk show to use app addon and engines with ember-cli-eyeglass.

This should be reviewed for:

accuracy
presentation
intended mental model of our users
etc. etc.
