Adding an entity directly to a chapter #2

I couldnt add any entity directly to my Chapter (using the + next to the chapter name)

This happened in Chapter 1 (http://editortool.linkedtv.eu/avro/8a8187f2-3fc8-cb54-0140-7dccd76f0001/) but worked in Chapter 2

This is also related to the issues about time entry when adding a new entity in a chapter.

Jaap - when adding a custom annotation to a chapter the start and end-times are automatically set to the start and end of the chapter. This way, the new annotation should always show up in the selected chapter. (Issue: Adding an entity directly to a chapter)
