Bump Prometheus to 2.3.2, Grafana to 5.2.2 #54

Modify Dockerfile and entrypoint script to leverage Grafana's dashboard and datasource provisioning. Also misc Kubernetes config updates.

Grafana dashboards updates, to better match Grafana 5.2.2 features, will come in a subsequent PR.

@siggy Ahh, whoops -- I just read your comment about the dashboard updates. Totally fine to address those in a separate PR.
