Update ingress documentation #98

[ ] Document l5d-dst-override (see linkerd/linkerd2-proxy#173 and linkerd/linkerd2#1998).
[ ] Update ingress-nginx with new annotation.
[ ] Add traefik documentation for ingress.
[ ] Link to service discovery architecture documentation (#97).

We should make sure that the docs for the l5d-dst-override header note that the header value is expected to be just a hostname, not a hostname and port (or scheme and hostname); it overrides the HTTP/1.1 Host header or HTTP/2 :authority pseudo-header, so it may only contain a valid authority.

The RFC links may be handy to link to in the docs as well?
