JBIG2 missing images (regression) #209

To reproduce, convert test files from 
https://code.google.com/p/pdfium/issues/detail?id=85 - e.g. control.pdf. Page 1 
is now mostly blank (besides a Google watermark).

Well, good thing my CL is split into 15 patch sets. Let's see where things went 
wrong...
