Wondering if index.js should just go away at this point and edit package.json to be "main": "lib/mean"

As I believe that there are not to much 0.3.x mean instances in the wild I think we're safe to remove it :)

@liorkesos The files it references for fall back actually don't exist anymore, which is why he felt so safe deleting it, so if there are any 0.3.x they're either pointing to an old version of meanio or yeah, they don't exist, lol
