Support for External Display Ports #27

Add support for external display ports. Most of this should already work, but it seems that HPD notifications from the USB-C port manager to the display port driver may be missing.

Depends on #35 for alt-mode/orientation/DP detection (at least for USB-C ports; DP via Surface Connect should work without this).
