[WIP] Thumbnail refactoring #196

Cleanup to make it easier to add/remove sizes, and then add a small thumbnail size and an x-large/xx-large size.

Testing because some of the upstream commits don't match up, and I need to do a lot of testing first.

With the likely upcoming Muffin rebase, I feel like holding this off.
