Clean up coverage config and output report directly to qa/coverage

Overview

• The index files don't do anything useful, but show up uncovered and mess with our stats.
• We want to include mjs an cjs as well as js.
• We can improve the testing by checking the config override

Deliverables

• [x] inclued .cjs and .mjs files
• [x] exclude index.cjs, index.mjs, and index.js files
• [x] fix the pattern for xxx.test.[cm]?js files
• [x] update output to qa/coverage
• [x] test the config override
