add breath_hold variable to data

Instead of creating two separate data sets, add a new variable (breath_hold) to the data that has the following levels:

TRUE if that observation is in Epoch 1, is for MAP ID 001-025, and comes from auto3TBH or man3TBH
FALSE if that observation is in Epoch 1, is for MAP ID 001-025, and comes from auto3T or man3T
NA otherwise

We have decided not to do this and to rather continue with the method of creating two separate data sets for FM and BH.
