A bit after the fact since we've recently opened many PR's in this repo but I thought it might be a good idea to add a PR template here as well similar to the one in go-livepeer.

@kyriediculous This PR looks good to me. I changed the target branch to streamflow since that is the current default branch for this repo which created some conflicts. Can you rebase this branch on top of streamflow to resolve those conflicts?
