Pediatric assoc availability fix #284

Fix for availability structure (issue #283).

After a rather thorough search, the only site offering vaccinations it the Tozer Rd., Beverly office.

Resources which confirm this are:

  1. https://www.salem.com/vaccine
  2. https://vaxfinder.mass.gov/locations/beverly-pediatric-associates-of-greater-salem/

I suggest that before this fix is merged, we change to reporting only the Tozer Rd, Beverly location. That means, we should remove the Highland Ave, Salem location from config.js.

@harcod : I was hoping we would tackle the issue of only reporting the Tozer Rd., Beverly location. From what I can tell, the Highland Ave location does not offer vaccinations.
