Add issue and pull request template. #17

As of now there is no issue or Pull request template. A proper template should be added for both in order to maintain smooth functioning.

I don't see any issue regarding Issue declaration and PR. We don't need a template.

@llaske If we use a template it will make it easy for other developers to go through issue and PR. Many open source communities use templates for smooth functioning. So I thought of creating an issue for the same. :)
