Custom blocks fixes #217

@llm-ui/markdown Markdown visible text no longer includes \n. This was causing behavior that was difficult to predict.

@llm-ui/react

useLLMOutput hook now returns all known blocks, it doesn't truncate the blocks based on how much is visible.
isVisible: true|false added to 'blockMatch' returned from useLLMOutput hook.

@llm-ui/custom

New options:
- defaultVisible: false,
- visibleKeyPaths: [],
- invisibleKeyPaths: [],
