So, does this do anything to change the question? Like, does this mean that the author doesn't have to specify any answer choices, or does it alter the structure of the question in some other way? Or is it just something we tack onto the data about the question in the model?
I think it is just something we tack on in the model. When an author specifies this, this will let us tell the PLS to present the student with an interface to draw an answer instead of (or in addition to) typing out an answer.

Some questions have answers that are figures. Right now, students have to describe those pictures in text, which is not cool. While Quadbase is not the place where a student enters an answer, an author should be able to state that a question's answer is a drawing. Then tools that do let students answer the question can present an appropriate interface.
