Type of map form control

Overview

Need to add a form control where user can specify what type of data the map is for. The types are:

• Entity: data relating to anything in the LIQ entity database
• Boundary/Thematic: data attached to an ABS boundary such as SA1, SA2, etc.
• Trade Area: data attached to a trade area

Upon selecting one or more map types, irrelevant controls are hidden.
