Content changed from referring physician to ordering physician/provider #165

The white paper uses the language referring physician. In the XML this was changed to ordering physician/provider. Unclear why this was changed, as with issue number 164. Additionally, this change is inconsistent with the change made in Issue number 164, introducing the possibility for confusion by adding another phrase refer to the same concept in the same order screen. This change was repeated three more times in the same action group, Lines 309, 315, 321.
