Duplicate KNART #445

This KNART needs to be eliminated. It is a duplicate of b17, which is a combination of 3 orders sets, created that way because it did not make sense to create them separately. The 3 duplicate order sets were created for contracting purposes.

@linophth b18 and b19 and been removed from /content. I've made all accompanying metadata and index changes.
