Make PTSD white paper changes as approved by VA SMEs. #473

The PTSD white paper has been approved for sharing by VA SMEs. Only the white paper has been reviewed - no related artifacts were reviewed. The attached Word document includes edits requested by the SMEs, with tracking turned on - this is the approved version. 20180506CDSK_CCWP_037PTSD_final_508_KBS_APPROVED_LLW.docx

@Fcuello No -- I think this one is waiting on changes from @Fcuello.

@a-dru-desai @linophth For posterity, this particular white paper XML file has 875 instances of tooling cruft that for whatever reason are cluttering up the version delivered to us. I am in the process of removing them, though am hoping this is a one-off instance and there are no other substantive oddities or differences going on between the delivered white paper XML and version reviewed by SMEs. @Fcuello I've fixed all the xxe stuff. A cursory review of the new PDF seems to include all the changes you made and there are no obvious content differences jumping out at me.
