When creating a character's backstory and statistics, add a section to assign permanent health-related disadvantages, stuff like heart-problems (caffeine highly increases chances of death), lactose intolerant (dairy products have negative health effects, may suddently increase bowel over time), insomnia (inability to sleep without pills), laziness (learning new skills takes much longer time, perhaps increased tiredness rate?), higher tendency to addictions (lesser amount of substance can get you addicted sooner and quicker, harder to get rid of them), introvert (with added stress meter that raises with each dialogue, smokes, alco, mushrooms and sauna to relief stress), etc.
