Possibly a duplicate of #801. This can be caused by loading a saved game from the main menu, and then immediately (before equipping the camera) opening the pause menu to load a saved game on the same level.

I don't see him doing what you just described, instead he moved to another level and returned back to the previous one at 02:20:30, then he loaded previous quicksave by pressing a hotkey at 02:22:20. I haven't tried to reproduce that myself, so idk if this is what caused the glitch. Hope that will help somehow.
