How do we mark the physical hole? Would that be a <gap @reason="damage">? It's not my intuition, as the hole has obviously always been there.
Should we add a @reason="physical" to the <gap> or is another element better?
Edit: I wonder whether <space> it better than <gap> as gap signifies some material that is left out, possibly because it is illegible.
