Added missing instructions for configuring the Apache .access file

Thanks for pointing this gap out and making the PR!

I'd rather not have to deal with questions on server setup, since that can get complex, so I've pointed back to CodeIgniter's user guide where they have much more detailed information on running the site. That's much more within the domain of the core framework than an addon.

Closing but look forward to other PRs from you!
