Fix PDF formatting in consent-video frame #335

In the consent-video frame, families have the option of downloading the consent form as a PDF. This button does produce a PDF version of the consent form contents, but some of the style/formatting is lost. Ideally the PDF would have some basic formatting to make it easier to read.

@ianchandlercampbell here's the issue about the PDF that's generated from the consent form. Feel free to post here if there's anything you want to add.
