with new model layout, create templates and views for forms

Did a lot of DB reorganization here. There's still quite a bit of work to do at this point.

I still need to

• [x] finish rewiring the client forms
• [x] create a broker form
• [x] connect the user tables to all the other forms (broker, lender, client)
• [x] update the views to write to this new multi model format
