LENDER FORM: Add the Broker Relationship Sub-form #26

Add a sub form called "Broker Relationship". move the field "Solicitation period" from the Lender Information, Move the field "pays Brokers fees" from the qualifiers sub form. Also add "Pay through Escrow", "Pays Broker rebates", and "Pays via 1099."
