Add clickable links from each service to Nagios #59

We have Graphite charts and problem resolution info on our Nagios pages.

This change to Nagdash makes service names clickable to go through to the backend Nagios instance service page so that operators can see more info about the actual problem.

The base URL of each Nagios is configurable in config.php (example updated)
