FastCGI cache

It should be possible to cache FastCGI responses if:

• it's an idempotent request (e.g. GET)
• there are no cookies
• there's no query string
• a setting has been turned on in the configuration file

This should help in popular cases such as wikis, blogs, and other mostly-read-only web sites.
