ad_sthlp - implement sthlp conversion to not rely on markdoc #5

This PR includes an update to ad_sthlp that includes a custom mdhlp to sthlp conversion.

This makes it possible to not have to rely on the external tool markdoc. markdoc is great but dependencies does not work great in Stata. And with our own conversion code we can make this conversion fit in the adodown ecosystem even better.
