add fax number to registered agent #200

Orignal subject: change phone to fax everywhere

It turns out that a fax number is required and a phone number is not. So let's replace all references to phone with references to facsimile.

Uh, it's not true that phone is not required. They are both required. See http://www.copyright.gov/onlinesp/agent.pdf for the exact fields.

On thinking about this more I don't know that info on our pages needs to be a precise mirror of the information in the agent registry.

Reassigning this to myself for further research.

Add fax field to /settings form.

Show fax in direct contact info form.

Add the markup. Now just waiting on a Git issue to finish styling and close this out.
