ability for a site admin to create a dispute ticket #209

A copyright owner posts a request by circumventing the form and either emailing, snail mailing, or faxing. Now the site admin has the request in hand. This person needs to be able to create a request by entering details by hand.

Feature: ability to add an attachment. The attachment is a URL. It is the faxed, mailed, or email original.

Feature: ability to not fill in the assertions and signature section. The idea is that a site admin is using the form to copy in data from a faxed, mailed or emailed document.

So this is a manual entry of the basic form as seen on the /box portion of the site?

Yeah.

The idea is to make it possible for site admins to use the form themselves, on behalf of copyright owners who have sent their complaint out of band, and to do with with the least amount of new engineering for us.

/myinbox now exists. It opens in a lightbox. The "Add a Site" button is now "Add a Dispute."

Next step, Paige will buff it.

Reopening and assigning to myself.

I want to massage the language in this page a bit to reflect that it's a site admin and not a representative of the copyright owner:

Change "Your full name" to something about the person who file the dispute. Maybe "full name of person who filed dispute." Or maybe make field name "Filer's Full Name" and add explanatory text that is that longer blurb - "Name of person who posted the dispute."
