Bulk operations

[x] process csv for bulk user adds
[x] add to top of accounts.csv for easy recording (done)
[x] create vm_conf
[x] add to existing cust context in dbn_voicemail.conf (done)
[x] process csv for bulk user removal
[x] remove users from accounts.csv and dbn_voicemail.conf
[x] log changes for troubleshooting and reversal if necessary
[ ] process csv for bulk user field changes
[ ] make changes to fields by name or DID
[x] log changes for troubleshooting or reversal if necessary

many of these were implemented with the addition of flag operations
