Refresh button already exist, isn't ?
It is just under "search box"...
But may be do you use a theme, hiding this toolbar (and so the button)?

If I click Subscriptions, there is a drop down option to ReFresh. It's a pain
to get to and I would like a little button up near the search box or anyway, so
it's a one click not a two click solution.

Click on above mentioned Refresh button should do the same (in 1 click).
Why did it not fit your needs ?

If you goto the default page, there is no referesh button.
http://www.google.com/reader/view/#overview-page The Refresh button I think
your thinking about only works for that active feed, not all feeds (like the
subscribe-refresh chlicks do).

There is a difference but they require the same solution.
The refresh that is current under the subscribe -> refresh, is like clicking
the refresh button on a feed to manually refresh the feed.
It won't help with the google issue unless there are way to double check the
google feeds api and isn't totally simple and seems outside the the reader+
scope.

OK, idea is little bit clear for me :)
Anyway, finding a proper solution to complete this task is really hard.
The main task is to fire the refresh function (hidden behing a dynamic popup
menu and in minified code)
I'm not sure I will be able to do it !
but if someone has ideas, write them here...

Just noticed a change in google reader. Now when you click on subscriptions
(not the drop down arrow linked to it) It does the full refresh.
So this requested option is no longer needed, thanks for looking into it
though.
