Executable OpenDental.exe is not created by compiling the source tree

What steps will reproduce the problem?
1. Those in the wiki GettingStarted page

What is the expected output?

Expected output is to have an executable file called 'OpenDental.exe' in
the build tree relative path of
build/mono-2.0.unix/opendental-4.7-debug/bin/ after typing 'make' in the
build tree root.

What do you see instead?

There is no such file generated by the existing source tree.

This bug is caused by two problems:
1) a missing file in the build tree relative path: OpenDental/OpenDental.build
2) an incomplete file in the build tree root: gnudental.build
