Support for MkDocs 1.0 #7

MkDocs 1.0 changed the configuration format and doesn't provide a pre-populated pages (now nav) list anymore (at least not in the on_config hook).

[x] Use nav property
[x] Use on_nav event that should provide access to pre-populated pages
[x] Remove title extraction feature since that's now done properly by MkDocs

Proper support for MkDocs 1.0 is implemented in v2.0.0 of the plugin
