Change unique constraint on SampleType #97

Currently SampleType has a uniqueness constraint on name & facility.

I would like to extend this to be name, molecularFormula & facility

Reason is to support the case where a sample with a common name (e.g. 'Steel') 
can have a variety of chemical compositions.
