ability to only allow teachers or admins to access a book #93

look at LTI role and if the user is a teacher or admin make them a subscriber to the book, otherwise don't and they'll get an access denied screen.

Test Plan:

Set the "Candela Lti Teachers Only" site setting for the book to 1
Launch to the book with a user who isn't already subscribed
It should be allowed to login or not based on the setting
