Text Fields Unavailable #2

When I click the plus sign to add cards, I can't edit the text fields; also it only shows the first text field, not all of them.

If I change it to the Basic note type, close the AAP window, then open it again, it will then show all text boxes/allow me to edit them. It only works for that single type of note, however.

@slipperysloo , could you provide a small sample deck with these problematic types of notes? It will help me to reproduce the bug.
