This PR updates the names and types of some fields in the locationsMetadata table.

Generalizes three device variables in preparation for #93
Removes references for the same variables from ISP specific fields (not needed with DeviceID)
Removes PostalCode
Adds ServicePlan, ServicePlanCost, ServicePlanCostUnits
