This PR:

Fixes bug where domain showed up instead of name of instances when adding a user
Defaults to "unnamed" when the instance has no name"

In testing, it seems as if the instance always shows up as 'Unnamed' in the frontend when the user is created, and then when that user is edited it is shown as unset. It can then be set by editing the user, after which it shows up correctly. Incidentally, 'name' is a required field and so instances will never be unnamed.
