Per-instance integration configuration #71

Integration configuration can only be injected via environment variables.

Should be possible to dd configuration per integration instance. Essential for multi-tenant and multi-project integrations, e.g. for TIMELD_GATEWAY_PREJOURNAL__USER, TIMELD_GATEWAY_PREJOURNAL__CLIENT
