Server bind address config #90

About the README, there is one in the works, with a pretty comprehensive description of the images, how to use them, and how to recreate them. I intentionally withheld it from this set of commits so the first external Gateway image could be created and published quickly. The README is a fast follow 😄.

@Angus-McAllister Should we merge this now, or wait for the docker READMEs?

@Angus-McAllister will you be making structural changes to /packages/gateway/README.md? I need to make changes to the same file in #50. (If you'll only be making detail changes then we're unlikely to conflict.)
