Investigate default controller buttons #154

With the latest stable release, it was found that the default guitar and drum 
controller buttons were not pre-configured.  If possible, at least the guitar 
frets should probably default to F1 through F5, with Enter being one of the 
strum buttons.

Also, the conflict detection seems to warn if multiple buttons for the same 
instrument are un-configured (set to "(none)").  The conflict detection should 
be updated to disregard a button that isn't configured.

The conflict detection issue was corrected, but I still don't know why the 
buttons aren't pre-configured anymore.  This may have been an intentional 
change.
